The following supporting documents are required during enrolment:

• A copy of the child's Birth Certificate
• A copy of the child's most recent Australian Childhood Register history statement, OR a copy of an excepted exemption form

Note:

It is important that we receive ongoing updates on the child's immunisation history. If otherwise, the child will risk exclusion from the Centre if an immunisable disease breaks out, as well as cancellation of your Child Care Subsidy (CCS) entitlements by Centrelink.
